How to get a card at the Creston-Dement Public Library

To apply for a Library card, you will be asked to fill out an application form as well as provide 2 forms of ID – one picture ID, and a document with your name and address listed. Examples of accepted ID are listed below.

Children under the age of 13 must be accompanied by a parent or legal guardian to receive a library card. Children under the age of 18 must be accompanied by a parent or legal guardian to sign a computer user agreement form in order to use the computers.

Library cards may be issued to:

  • Residents of the Village of Creston or Dement Township are eligible for a free library card with verification of their current address and a photo ID. Resident library cards are valid for 2 years.
  • Individuals that live outside of the Village of Creston or Dement Township, but own property there, are eligible for a library card with a current tax bill for the property along with a photo ID.
  • Non-residents are eligible for a Creston-Dement Library card if they reside in an unserved, untaxed area for a fee, which is required by Illinois statute. The current annual fee for non-residents is $115.00. This card is valid for all members of the family residing at that location for a 12 month period.

  • Bill, mail or pay Stub
  • Checkbook (must include name and address)
  • Insurance or Medicare Card (Must have pre-printed address)
  • Lease / Mortgage
  • Voter Registration Card
  • Auto Insurance Card/Vehicle Registration
